Piles, Fissure & Fistula Care

Structured proctology assessment for common anorectal conditions.

Bleeding, pain, swelling, itching or discharge around the anus can have several causes. Haemorrhoids (piles), anal fissure and anal fistula are different conditions, and treatment depends on identifying which problem is present rather than treating every symptom in the same way.

Piles, fissure and fistula are not the same condition

Haemorrhoids are enlarged vascular cushions in or around the anal canal. An anal fissure is a tear in the lining of the anal canal and commonly causes pain during or after bowel movements. An anal fistula is an abnormal tract that can develop between the anal canal and the skin, sometimes after an abscess or infection.

How symptoms are assessed

Evaluation starts with the history of bleeding, pain, bowel habits, swelling or discharge and an appropriate examination. The clinician may advise additional examination or tests when symptoms are persistent, recurrent, atypical or associated with other warning signs.

Conservative and procedural treatment

For some patients, bowel-habit measures and treatment aimed at avoiding hard stools are part of early management. Medicines or local treatments may also be used depending on the diagnosis. Procedures or surgery can be considered when symptoms are persistent, severe or related to a fistula, significant haemorrhoids or another condition requiring intervention.

Why rectal bleeding should be evaluated

Bleeding should not automatically be assumed to be piles. Persistent or recurrent rectal bleeding, unexplained weight loss, marked change in bowel habits, anaemia, fever or other concerning symptoms need medical assessment so that alternative causes are not missed.

When urgent care may be needed

Heavy bleeding, severe worsening pain, high fever, rapidly increasing swelling, fainting or significant weakness require prompt medical attention. A painful swelling with fever may indicate infection or an abscess and should not be delayed.
